Disclaimer

The Proof Book provides organizational templates for record-keeping only and is not legal, tax, or financial advice. Consult a qualified attorney or accountant about your specific situation. The Proof Book is a trade name of The Chalk and Ledger Group LLC.

In plainer words

The templates help you write down money that already moved. They do not tell you what should happen next. They do not tell you whether a gift is allowed, whether to pay yourself for care, which account to spend, how to qualify for Medicaid, or what your state's power of attorney statute requires of you.

Those questions belong with a qualified elder-law attorney who can read your document and your state, and tax questions belong with a CPA. A template can make that meeting shorter, but it cannot replace it.

Where the stop signs are

Inside the POA Ledger, gifts and transfers have their own page on purpose. The page is a reminder to talk to an elder-law attorney before money moves that way. The caregiver-hours log is a record of time, not permission to pay a family caregiver. The Five-Year Summary holds sixty months so you can hand an organized file to an attorney. It is not a Medicaid application or a plan.

No promised outcome

We do not promise a particular response from a sibling, a bank, an attorney, a court, or Medicaid. Organized, honest records are useful, but they are not a guarantee.

Your entries are yours

You are responsible for the truth of what you type, and a neat ledger full of false lines is still false. We never see the bank file if you keep it in your own sheet, and we do not review your books.
